Loud Noise (t-case? trans?) HELP!!!!

I was driving home and all of a sudden I heard a loud continued clunk coming from the transfer case. I slowed down but so long as the truck was moving, I heard the noise and felt the truck vibrate like I had a flat. I could also feel the shifter handle move every time I heard the clunk. Sounded like the transfer case. Felt and sounded like the noise was coming from under the shifter. Crawled home at 2-3 mph. Once I went faster, I would hear the noise. Only stopped when the wheels stopped turning. I could even hear the noise and feel the clunk in the shifter when coasting in neutral. Heard more of a grinding sound when placed in reverse.

Any ideas? Where should I check 1st?

I got the truck on a lift and found that the 4-point cardan joint snapped. Do I have to replace the entire prop shaft or just the 4-point cardan joint?

Where can I buy these joint? How about the entire prop shafts?

You can call these guys and get a heavy duty unit that bolts in. Never worry again.
